Appointments

• Please call our office staff to schedule or change an appointment or speak to your physician or a member of our medical staff. • For cancellations, please call at least one day in advance to reschedule.

Fees

• All fees are incurred in our office are due and payable at the time of service. Our fees are within customary ranges and reflect the services and care you receive from us. Each month you will receive a statement, which will show an itemization of the charges incurred and any payments made by you or your insurance company.

• If you have questions regarding your billing, please call our Billing Office at (520) 439-4441. Please have your physician’s name, insurance company and account number ready.

Insurance

• Please bring your insurance identification cards and any claim forms provided by your insurance company to your fist visit

• It is important that you notify us if your insurance company requires authorization or pre-certification prior to treatment or hospitalization to avoid any reduction in your benefits

• If your insurance coverage changes for any reason, please advise our office as soon as possible.

Prescriptions

• To ensure you receive the appropriate medication, refills of prescription drugs can only be written during regular business hours when we have access to your complete medical records.
